IMPORTANT MEDICAID INFORMATION

Important announcement for HealthChoice Illinois - Medicaid beneficiaries:

Pana Community Hospital, Quad County Home Care Services, Community Medical Clinic and Pana Medical Group including all offices in Pana, Assumption, Nokomis and Ramsey currently accept the following HealthChoice Illinois - Medicaid Health Plans:

BLUE CROSS COMMUNITY FAMILY HEALTH PLAN

HARMONY HEALTH PLAN

ILLINICARE HEALTH PLAN

MERIDAN HEALTH PLAN

MOLINA HEALTHCARE PLAN

Some of these health plan directories have not been updated to include our hospital, clinics, physicians, or home care services.  We are working closely with these health plans to ensure their directories are updated.  Even though we are currently not listed on some of these directories, we are participating providers for all the HealthChoice Illinois - Medicaid health plans listed above. 

Until HealthChoice Illinois - Medicaid directories are updated you may not be able to select the primary care provider (PCP) or health plan of your choice.  Once you select a PCP and a health plan -OR- if you are automatically assigned a PCP and a health plan remember you will have 90 days to change your health plan and you may change your PCP at anytime.  All of Pana Community Hospital's providers and services should be incorporated in these directories within this 90-day time frame.
