Pana Community Hospital, Community Medical Clinic and Quad County Home Health and Hospice are currently seeking a Licensed Social Worker. The primary responsibility is to provide clinical counseling at the Community Medical Clinic. You will assist in providing psychosocial assessments for Home Health and Hospice patients. The applicant must have excellent communication and counseling skills and be able to facilitate the coordination of care with the patient, family and interdisciplinary team. This individual will assist the team members in understanding the significant social and emotional factors related to the health challenges, participate in the plan of care, prepare clinical and progress notes and have a general knowledge to make appropriate referrals to community resources. Must have a Master's degree in Social Work (minimum LSW); current Illinois license (LSW) is required; and a minimum of 2 years experience in the health care field, as demonstrated by current competence. Hours will be Monday-Friday, 8:00am-4:30pm.

BRIEF DESCRIPITION: This individual's responsibilities include: patient registration, scheduling, receptionist and switchboard, insurance preauthorization, verification of benefits, document scanning and indexing.
QUALIFICATIONS: High school graduate or equivalent (GED); above average computer skills; familiarity with Microsoft applications including Outlook, Word, and Excel; excellent verbal and written communication skills; CPT-4 and ICD10 coding experience preferred; able to perform assigned duties independently without direct supervision; able to assist with flexible hours in order to meet departmental staffing needs; calculator, fax, and copier experience required; and previous experience working with public required, preferably in a healthcare setting.
WORK SCHEDULE: Monday-Friday 9:30 am - 6:00 pm; occasional 7:00 am - 3:30 pm, Saturday rotation 7:00 am - 12:00 pm.
