WORK SCHEDULE:  6:00 AM - 6:00 PM

BRIEF DESCRIPTION:  A professional registered nurse who is responsible for planning the care of the medical/surgical or critically ill pediatric, adolescent, adult or geriatric patient, based on the assessment of the patient’s subjective and objective health care needs, and to coordinate the nursing care plan with the medical regime established by the primary physician.  The registered nurse is expected to possess the depth of knowledge required to plan, implement, and evaluate the effectiveness of such care according to the Nursing Department’s standards of care and practices.

QUALIFICATIONS: Graduate of an accredited school of nursing; current Illinois license as a registered nurse; knowledge of trends in nursing and health care as demonstrated by current competence; current CPR certification; encouraged to belong to one professional organization within current scope of practice/specialty; completion of appropriate skills checklist within 90 days of employment/annual evaluation; willing to work flexible, alternate, and/or additional shifts if warranted; and maintain ACLS/PALS minimum, or advanced skill certification within 6 months of employment for ED assignments.

BRIEF DESCRIPTION:  The Exercise Physiologist will be responsible for performing routine fitness assessments and overseeing exercise prescription and adherence.  This role will also be responsible for monitoring and reporting exercise safety and program design for the department.  the Exercise Physiologist will also provide clients with an orientation to equipment and fitness programming.  This role will also be involved in instruction of group exercise classes, personal/group training, and health coaching.

QUALIFICATIONS: Bachelor’s degree in Exercise Physiology, or related field, Master’s preferred; CPR certification; Personal Trainer Certification (ACE, ACSM, NSCA, NASM); Health Coach Certification, NCCA accredited; a passion for and active in health, wellness and fitness pursuits; proficient with computers, data management, Microsoft, etc.; and ability to read, speak, write and follow written and oral instructions.

BRIEF DESCRIPTION:  The Respiratory Care Practitioner is responsible for implementing the plan of care for those individuals who have experienced an event or have disease processes related to their cardiac and/or pulmonary systems.  The respiratory care practitioner is expected to possess the depth of knowledge required to implement and evaluate the effectiveness of such care according to the standards of

care and practices.

QUALIFICATIONS: High school graduate or equivalent, with an Associate of Science (A.S.) degree (preferred); graduate of a Joint Review Committee for Respiratory Therapy Education (JRCTE) approved program of respiratory care required; licensed by the Illinois Department of Professional Regulation as a Respiratory Care Practitioner (RCP); twenty-four (24) months recent and relevant experience in cardiopulmonary practice settings; current CPR and ACLS certification; demonstrates ability to perform necessary skills and competencies defined by the skills checklist for Cardiopulmonary Rehab; commitment to a healthy lifestyle as a model for participants and staff; and willing to work flexible, alternate, and/or additional shifts if warranted.

BRIEF DESCRIPTION:  The primary responsibilities of this position include:  patient registration, scheduling, receptionist and switchboard, insurance preauthorization, verification of benefits, document scanning and indexing, payment receipts.

QUALIFICATIONS:  High school graduate or equivalent (GED); excellent verbal and written communication skills; above average computer skills; familiarity with Microsoft applications, including Outlook, Word, and Excel; able to perform assigned duties independently without direct supervision; able to assist with flexible hours in order to meet departmental staffing needs; previous experience working with public required, preferably in a healthcare setting; and CPT-4 and ICD10 coding experience preferred.

BRIEF DESCRIPTION: This individual will be providing assessments of social, financial, resources and emotional needs of patients in families in Quad County Home Health, Quad County Hospice and the Community Medical Clinic; supporting and educating patients and families regarding end of life issues and advanced care planning; participation in Hospice Interdisciplinary team meetings; and works in conjunction with hospice bereavement coordinator to assess patients for bereavement needs and transition to bereavement care.

QUALIFICATIONS:  Preferred candidates must be a Licensed Clinical Social Worker or have a valid Master's in Social Work degree and actively pursuing their LCSW.
